Terms & Conditions
Patient Terms and Conditions
Effective Date: 10/28/26
This Patient Terms and Conditions Agreement ("Agreement") is between AgentDDS.com, LLC, an Oregon limited liability company doing business as OneNumb.com ("OneNumb," "we," "us," or "our"), and the individual accessing or using OneNumb to search for, request, schedule, or manage a dental appointment ("Patient," "you," or "your"). By accessing or using the Platform, and where required by affirmatively accepting these Terms, you agree to be bound by this Agreement.
1. Definitions
- "Platform" means OneNumb.com and any related applications, telephone-assisted or AI-assisted scheduling services (including HelloAnnie), or other services through which OneNumb facilitates dental appointment discovery or scheduling.
- "Dentist" or "Office" means the participating dental professional, practice, or provider with whom you schedule or request an appointment through OneNumb.
- "Services" means the appointment-search, scheduling, communication, and related facilitation services provided through OneNumb.
- "Cancellation/No-Show Fee" means the $50 fee an Office may charge when a Patient fails to attend a scheduled appointment or fails to cancel by the applicable deadline in Section 4.
- "Standard Booking" means an appointment booked four (4) or more hours before the scheduled appointment time.
- "Short-Notice Booking" means an appointment booked less than four (4) hours before the scheduled appointment time.
- "Telephone-Assisted Booking" means an appointment requested or scheduled by telephone through OneNumb or a third-party service provider assisting OneNumb, including HelloAnnie.
- "Stripe" means Stripe, Inc., the third-party payment processor. "Stripe Connected Account" means an Office's merchant account with Stripe through which the Office may initiate and receive an authorized Cancellation/No-Show Fee.
- "PHI" means Protected Health Information as defined under HIPAA.
2. Nature of the Services
The Platform is intended for non-life-threatening dental needs only and is not for medical or dental emergencies. If you are experiencing severe swelling, difficulty breathing, uncontrolled bleeding, fever associated with a dental condition, or any other potentially life-threatening emergency, call 911 or go to the nearest emergency department.
OneNumb is a scheduling and appointment-discovery platform that connects Patients with participating Offices. OneNumb does not provide dental care, diagnose conditions, provide medical or dental advice, determine what treatment is appropriate, guarantee that a provider is appropriate for your needs, or determine insurance coverage or benefits. Any examination, diagnosis, treatment, or clinical care is provided solely by the Dentist or Office.
3. Booking and Payment-Card Authorization
Emergency notice: If you have a life-threatening emergency, call 911. Do not use OneNumb.
Online Bookings
You are not charged at the time of booking solely for saving a payment method. To complete certain online bookings, you may be required to provide a valid payment card. Card information is processed and stored by Stripe using a tokenized credential; OneNumb does not store your full card number. By completing a booking that requires payment authorization, you: represent that you are authorized to use the card; authorize the applicable Office to retain the tokenized credential through Stripe for permitted purposes; authorize the Office to charge an applicable $50 Cancellation/No-Show Fee as described in Section 4; and understand that saving a card does not itself result in a charge and that any fee is initiated by the Office, not automatically by OneNumb.
Telephone-Assisted Bookings
A Telephone-Assisted Booking does not, by itself, authorize OneNumb or an Office to charge a payment card. If payment authorization has not been completed when the booking is created, OneNumb or the Office may contact you to complete it through an approved OneNumb, Stripe, or other authorized payment workflow. The Cancellation/No-Show Fee may be charged only where the required authorization has actually been obtained from you or an authorized adult payer. Where Terms, authorization, or other disclosures have not yet been affirmatively accepted, OneNumb or the Office may require additional steps before those provisions apply.
4. Cancellation and No-Show Fee
$50 fee, charged by the Office for qualifying late cancellations or no-shows where valid payment authorization has been obtained.
If you fail to attend your scheduled appointment or fail to cancel by the applicable deadline, the Office may charge a $50 Cancellation/No-Show Fee to the authorized payment method associated with the booking. The fee is part of OneNumb's booking policy and is intended to protect participating Offices against missed appointments and qualifying late cancellations.
- Standard Booking: cancel at least four (4) hours before the appointment to avoid the fee.
- Short-Notice Booking: you have one (1) hour from the time you book to cancel without a fee. Any cancellation must occur before the appointment begins. If the appointment starts less than one hour after booking, the cancellation period ends when the appointment is scheduled to begin.
- Attend as scheduled: no fee applies.
- No-show or qualifying late cancellation: the Office may charge the $50 fee where authorized and lawful.
The Office is responsible for determining whether the fee applies, initiating the charge, deciding whether to waive, reduce, refund, or adjust it, and complying with applicable laws and payer requirements. The Office may waive or refund a fee at its discretion. OneNumb does not automatically determine or impose fees.
Disputes and Refunds
If you believe a fee was charged in error: (1) contact the Office directly within seven (7) days of the charge to request review, correction, or refund; (2) if the Office does not respond, contact OneNumb at support@onenumb.com for informal assistance; (3) if unresolved, you may contact your card issuer. Because the Office initiates and receives the charge, the Office decides whether a refund or adjustment is appropriate. OneNumb may try to facilitate communication but does not control the Office's final decision. The cancellation deadline and the seven-day dispute period are separate.
5. Office Billing, Insurance, and OHP
Dental-service charges. OneNumb does not bill or collect payment for dental examinations, x-rays, treatment, or other clinical services. All charges for dental care are determined, billed, and collected directly by the Office, which is solely responsible for its fees, billing policies, insurance participation and claims, eligibility and benefit verification, network-status verification, Patient balances, and treatment financial agreements. OneNumb may charge participating Offices fees for use of the Platform or for appointments facilitated through OneNumb; those fees are obligations of the Office and are not charged to the Patient.
Insurance information. OneNumb may collect insurance information during booking and display insurance information provided by Offices. A statement that an Office "accepts" or "participates with" a carrier, or lists a carrier or plan, is not a guarantee or verification that the Office is in-network with your specific plan, that your coverage is active, that a service will be covered, that your insurer will pay any particular amount, or that you will not owe a deductible, copayment, coinsurance, or other amount. Eligibility, benefits, and network participation must be verified by the Office and/or your insurer, and you are responsible for confirming important insurance information before treatment.
Oregon Health Plan (OHP). OneNumb may ask whether you are enrolled in OHP. Depending on the Office, your OHP status, assignment, Open Card status, or other circumstances, OneNumb may not permit an OHP-related appointment to be completed through the Platform, and you may be directed to contact the Office directly. OneNumb does not determine or verify OHP eligibility, enrollment, assigned dental provider, Open Card status, benefit coverage, or whether an Office may provide covered services to you. The Office remains responsible for verifying OHP information and complying with Oregon Medicaid requirements. Where law or payer requirements prohibit a missed-appointment, cancellation, or similar fee for an OHP member, no such fee may be assessed through OneNumb. Nothing in these Terms replaces any separate financial or private-pay agreement an Office may be legally required to obtain.
6. Privacy and Data Protection
OneNumb is committed to protecting personal information and handling it in accordance with applicable privacy and data-protection requirements and our Privacy Policy at onenumb.com/privacy-policy.
Information we may collect: name, email, phone number, date of birth or age where necessary, appointment and scheduling information, Office selected, appointment date and time, insurance information provided during booking, OHP status where applicable, information you submit when communicating with OneNumb or an Office, payment-method information processed through Stripe, and technical and usage data (IP address, browser, device, Platform activity). Depending on how you use the Platform, some information may constitute health information or PHI.
How we may use information: to display availability; facilitate bookings; send confirmations, reminders, cancellations, and scheduling notices; request completion of payment authorization or other booking information; exchange scheduling information with participating Offices and connected systems; support insurance-related screening or Office follow-up; facilitate card authorization and permitted Cancellation/No-Show Fees; provide support; maintain and improve the Platform; detect errors, fraud, abuse, or security issues; and comply with legal obligations.
Service providers and sharing. OneNumb does not sell personal information. We may disclose information when reasonably necessary to operate the Platform, including to participating Offices; HelloAnnie (Annie Labs) and connected practice-management systems; Stripe; Twilio; Resend; and hosting, analytics, security, support, and infrastructure providers. We may also disclose information where required by law or reasonably necessary to protect the Platform, Offices, users, or others. If you join an Office waitlist, information needed to identify and contact you may be shared with that Office.
Security and retention. OneNumb uses reasonable safeguards, but no electronic system is completely secure. Information may be retained as reasonably necessary to provide the Services, maintain records, resolve disputes, comply with legal and contractual obligations, maintain security, and enforce agreements.
Privacy rights. Depending on applicable law, you may have rights to access, correct, delete, or obtain a copy of certain information. Requests may be submitted using the contact information in the Privacy Policy. Certain information may be retained where required or permitted by law, including information maintained by Offices as healthcare or business records.
7. Liability and Disclaimers
OneNumb is a scheduling and booking platform and is not a healthcare provider. It does not provide medical or dental advice, diagnosis, treatment, or clinical recommendations, and use of the Platform does not create a dentist-patient relationship with OneNumb. OneNumb does not guarantee the quality of care provided by an Office, the outcome of treatment, that an Office is appropriate for your needs, that any appointment will remain available until booking is completed, insurance coverage or network status, or uninterrupted availability of the Platform.
To the maximum extent permitted by law, OneNumb's aggregate liability to you for claims arising out of or relating to the Platform, Services, or this Agreement will not exceed [$100 per Patient – counsel to confirm amount and enforceability].
OneNumb is not responsible or liable for clinical care provided by a Dentist or Office; Office billing practices, treatment fees, insurance determinations, refund decisions, or Cancellation/No-Show Fee decisions; charges initiated by an Office; errors or delays caused by Offices or third-party services; practice-management system errors; HelloAnnie errors or interruptions; Stripe, Twilio, Resend, or other service-provider failures; or availability that becomes inaccurate because of scheduling activity, synchronization delays, connectivity problems, or other technical issues. Nothing in this Agreement limits liability that cannot lawfully be excluded or limited.
8. Patient Conduct and Responsibilities
You agree to use the Platform only for lawful purposes and not to: submit false, misleading, incomplete, or fraudulent information; use a payment method you are not authorized to use; book appointments you do not genuinely intend to attend; impersonate another person or misrepresent your identity or authority; interfere with the security or operation of the Platform; attempt unauthorized access to the Platform or connected systems; or use the Platform in violation of law. You are responsible for providing accurate contact, patient, insurance, and scheduling information and for reviewing confirmations, reminders, payment-authorization requests, and other communications about your appointment.
9. Governing Law and Dispute Resolution
This Agreement is governed by the laws of the State of Oregon without regard to conflict-of-law principles. Except where prohibited by law, disputes that cannot be resolved informally will be resolved through binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, individually and not as a class, consolidated, or representative proceeding except where law requires otherwise. Unless otherwise agreed or required by AAA rules, arbitration will take place in Portland, Oregon, and may be conducted remotely. Nothing prevents either party from bringing an eligible claim in small claims court or seeking emergency or injunctive relief where permitted by law.
10. Acceptance of Terms
Online bookings. By checking the box indicating agreement and completing an online booking, you acknowledge that you have read, understood, and agree to this Agreement, including payment-card authorization, Cancellation/No-Show Fee terms, electronic communications, and dispute-resolution provisions. If you do not agree, do not complete an online booking.
Telephone-Assisted Bookings. A Telephone-Assisted Booking may require separate electronic or other affirmative acceptance of these Terms, payment authorization, or other disclosures. A telephone scheduling interaction alone does not constitute payment-card authorization. Where separate acceptance or authorization is required, the applicable provision becomes effective only after it has been obtained.
11. SMS/MMS Messaging
OneNumb may send SMS/MMS messages to the mobile number you provide, subject to applicable consent requirements and your messaging preferences. Transactional messages (appointment confirmations, reminders, availability alerts, cancellation or scheduling notices, requests to complete payment authorization, account verification, and support) relate to an appointment, account, or request you initiated. Marketing messages are sent only if you separately opt in; marketing consent is optional and not required to book or use the Platform. The full terms of our text messaging program, including message frequency, rates, and how to opt out (reply STOP) or get help (reply HELP), are in the OneNumb SMS/MMS Terms and Conditions, which are incorporated into this Agreement. If these Terms and the SMS/MMS Terms conflict about text messaging, the SMS/MMS Terms control.
12. Appointment Availability, Booking Confirmation, and Technical Issues
Availability displayed through OneNumb is obtained from participating Offices and connected scheduling systems. OneNumb may use third-party technology and integrations, including HelloAnnie and connected practice-management systems, to retrieve availability and create or manage appointments. Availability may change because of Office scheduling activity, bookings through other channels, synchronization delays, connectivity problems, duplicate booking attempts, third-party outages, or technical errors. A time displayed as available is not guaranteed to remain available until booking is completed, and OneNumb may revalidate availability before confirming a booking. An appointment is considered booked when the booking process is successfully completed and a confirmation has been issued, subject to correction of technical or scheduling errors.
For Telephone-Assisted Bookings, an appointment may be placed on the Office's schedule before payment authorization is completed, and may be treated as scheduled with incomplete payment authorization until the remaining steps are completed. OneNumb and Offices may correct scheduling errors or contact you if an appointment cannot be honored, and reasonable efforts may be made to notify you and help with alternative scheduling. OneNumb does not guarantee uninterrupted access or error-free appointment information.
Rescheduling. Rescheduling may be treated as cancellation of the original appointment for purposes of the Cancellation/No-Show Fee. If an appointment is rescheduled after the applicable cancellation deadline, the Office may determine that the fee applies where authorized and lawful.
13. Bookings for Other People and Patients Under 18
You may schedule for yourself or another person if you are authorized to do so, and you represent that you are authorized to schedule on that person's behalf and to provide the information submitted, and that it is accurate. OneNumb requires an individual age 18 or older to complete the booking process for any Patient under 18. The adult completing the booking represents that they are at least 18; are authorized to submit the Patient information; are authorized to use the payment method provided, if applicable; and authorize any applicable card storage and Cancellation/No-Show Fee under Sections 3 and 4. This is a OneNumb platform and payment policy. It does not determine whether a minor may independently consent to dental diagnosis or treatment under applicable law. Each Office remains solely responsible for determining who may legally consent to treatment, obtaining any required clinical consent, and determining whether a parent, guardian, or other authorized individual must participate in treatment decisions.
14. Dentist and Office Information
Participating Dentists and Offices are independent healthcare providers, not employees, agents, partners, or representatives of OneNumb. The appearance of a Dentist or Office on OneNumb is not a guarantee regarding that provider. Office information (services, availability, hours, location, phone number, insurance participation, and other practice information) may come from the Office or connected systems, and OneNumb does not guarantee it is complete, current, or error-free. You are responsible for confirming important information directly with the Office, including insurance participation, treatment availability, treatment costs, and whether the Office is appropriate for your needs.
15. Office Cancellations and Schedule Changes
A Dentist or Office may cancel, modify, or reschedule an appointment because of provider availability, scheduling needs, emergencies, technical issues, or other circumstances. OneNumb does not control these decisions. A Patient will not be charged a Cancellation/No-Show Fee solely because the Office cancelled or changed the appointment. To the maximum extent permitted by law, OneNumb is not responsible for expenses or losses caused by an Office cancellation or schedule change, including travel expenses, lost wages, missed work, childcare expenses, or delays in treatment. OneNumb or the Office may use your contact information to communicate schedule changes and, where possible, assist with alternative scheduling.
16. Electronic Communications
By providing an email address or mobile number, you agree that OneNumb and participating Offices may send transactional electronic communications related to the Platform and your appointment, subject to applicable consent requirements. These may include booking confirmations, reminders, cancellation and rescheduling notices, requests to complete payment authorization or other booking requirements, support communications, payment or Cancellation/No-Show Fee notices, security or account messages, and updates to these Terms or the Privacy Policy, by email or SMS/MMS. You are responsible for providing accurate contact information and reviewing communications. OneNumb is not responsible for messages not received because of incorrect contact information, spam filtering, carrier problems, outages, or other circumstances outside its reasonable control. Marketing communications are governed by the SMS/MMS Terms.
17. Suspension and Termination of Platform Access
OneNumb may restrict, suspend, or terminate access to all or part of the Platform where reasonably necessary to protect OneNumb, participating Offices, users, or the Platform, including for fraudulent bookings, repeated bookings without genuine intent to attend, fraudulent payment activity, security threats, harassment or abusive conduct, or violations of these Terms or law. Suspension or termination does not automatically cancel an existing appointment, and you remain responsible for contacting the Office to cancel or modify it.
18. Changes to These Terms
OneNumb may update these Terms to reflect changes to the Platform, Services, business practices, legal requirements, or operational processes. The current version will be available through OneNumb and will identify its effective date. Where changes are material, OneNumb may provide additional notice or require affirmative acceptance before continued use of certain features. Changes apply prospectively unless law requires otherwise.
19. Miscellaneous
Severability. If a provision is invalid, illegal, or unenforceable, it will be enforced to the maximum extent permitted and the rest remains in effect. No waiver. Failure to enforce a provision does not waive the right to enforce it later. Assignment. You may not assign or transfer your rights or obligations without OneNumb's prior written consent; OneNumb may assign this Agreement in connection with a merger, acquisition, reorganization, financing, asset sale, or similar transaction, subject to law. Entire agreement. This Agreement, together with policies and notices expressly incorporated into it (including the SMS/MMS Terms and the Privacy Policy), is the agreement between you and OneNumb regarding use of the Platform and supersedes prior understandings on the same subject. Headings are for convenience only.
